Records release forms may be submitted via fax 843-524-0425, by email at [email protected] or in person.
Sports Physical Forms , Medication Forms and Head Start Well Child Forms must be printed, the parent portion must be completed and the original dropped off for the doctor to complete.
Once the forms have been received by our office, please allow atleast 48 - 72 hours for completion.
