EVALUATION PACKAGE
For New ADHD Evaluations:
Please complete anxiety form
(Teens can fill this out for themselves)
Please complete 2 parent forms (These can be done by any caregiver that spends a lot of time with child)
Please complete 2 teacher forms (These can be done by 2 teachers, or a teacher and an assistant)
Once forms are filled out, please bring to the office to schedule your child's evaluation appointment.
